Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It's about connecting families, communicating with healthcare providers, and showing your community what it means to care for one another.

In this discussion, Alexis shares what Five Wishes is. In general terms, the wishes are:
        1) Who is your healthcare agent
        2) What type of medical treatment do you want
        3) Comfort level - pain meds, how conscious you want to be, etc
        4) How do you want to be treated (faith, visitors, etc)
        5) Legacy
This is a very general overview. The document guides conversations around each wish. It really helps individuals and families think things through before a crisis occurs. The whole point of completing Five Wishes is to "protect human dignity". Wishes 3 through 5 are the "living will heart and soul".
If you are over the age of 18 you need to complete your plan. If you are a legal guardian, complete a document for your ward. This is especially important (IMO) if you are also the parent. Because when you are gone it's going to be this document and a letter of intent that will be available. Those who come after you will have to rely on these documents to make the best choices for your child.
